Patient Care / Medical Questionnaire
We encourage all of our new patients to fill out and submit our secure online medical questionnaire prior to their first appointment. The questionnaire is not required to request an appointment however submitting it before will expedite the appointment making process and allow Dr. Lacombe time to review your medical history and tailor your first appointment to meet your individual needs.
